APPLICATIONS MUST BE RECEIVED AT LEAST 10 DAYS PRIOR TO THE PREFERRED AUDITION DATE.   IF THE DEADLINE FOR APPLICATIONS CANNOT BE MET, PLEASE CALL FOR A REQUEST FOR  WAIVER UNDER CERTAIN CIRCUMSTANCES FORM.
